It doesn’t do anything particularly worthwhile. There were some people (wrongly) saying that it helped against laggy opponents, but it doesn’t and it never did — that is, cl_smooth smooths the movement of the client’s viewport if there is a prediction error. It does NOT help against laggy opponents.
