MW3 to charge monthly subscription fee

Correct.
They said 256 is technically possible but not fun to play.
According to their research the most fun games are with players between 32 and 40.
Due to this reason, BF3 won’t go above 64 players ( PC ).

bf servers never exceeded 64 slots anyway
the bf2’s maps were huge but it was already very annoying to play with 63 mate…..
a 64 slot serv already cost 120€ at least , i dont know who will pay a 256slots servers
( excepted teams with 20+ members :p )
Just to point out, most people already pay for XBL anyway without complaint, what makes you think people will be up in arms about this?
Honestly, let them run away with this. Fools and their money will soon be parted. Call of Duty was already a horrendous game. COD4 on PC was pretty decent, but the console version was a cesspit and it’s just got progressively worse with every title since.

There is still a difference between the console and PC generation. But your point is still valid though. It still hurts to see the cashflow going in the ( in my eyes ) wrong direction.

MAG uses 256 players and I think it works quite well. Maps are huge though.

realistic = get shot once, die or go to hospital for 2 month, get discharged, fly home, suffer from PTSD.
hyperrealistic = get shot 5 times, duck behind chest-high wall and just breath it off.
I hate this new breed of shooters.
