FPS problems

Since the 18th Sep Update I cant play TF2 at all. Game almost crawls.
Was getting a good 120 fps before.
Running Highframes config of Chris on a custom TF2mate config.
Any help would be appreciated.
try defrag? and make sure AV is either off or has whitelist for steam folder
Redownload overnight, reinstall graphics drivers, reinstall windows, whatever it takes man, to get that sweet sweet 120 fps..
tried defragging (with steam and perfect disk) and av whitelist is already there.
but still dropping to 40 fps
