Ok, so I run these “-novid -dev -console -32bit -windowed -h1080 -w1920 -noborder” with a dual monitor setup on Windows 7.
Sometimes my tf loses focus, i.e. my mouse pointer isn’t restricted to the main monitor anymore. I don’t really know how this is happening, except it’s usually when I move the mouse quickly and a lot of things are going on, i.e. I’m probably pressing several buttons on the keyboard. But haven’t been able to reproduce it. There are no notifications or anything from windows as far as I can see.
Any ideas/help would be appreciated.

just lol at those launch options.
WM_MOUSEMOVE is a crappy mechanism that has flaws like this. Try m_rawinput 1.

yeah I know some of them are outdated, just too lazy to look it up…changed it so you don’t have to lol anymore
using m_raw already though

yeah, this happens for me too when I’m using a dual-monitor setup. Easiest way to reproduce it is to flick the mouse to the other monitor and click.
No known fix for other than disabling the second monitor if you’ve got the same issue, afaik.
This has happened to me too. Not quite sure if some button causes the mouse to unbind from the TF2 window, but it seems to only occur with flick shots…
disable secon monitor during games
You could use a program to lock the mouse to one monitor when the game is launched, http://www.softpedia.com/get/System/System-Miscellaneous/MultiMonitor-Cursor-Lock.shtml not sure if it is the best but it was the first I found
Running multiple monitors when in windowed mode causes fps jitter anyway. You should turn off the second monitor in your driver settings software When you launch the game. Set up a profile so it automated.
