Weird Lag

Sooooo i used to play TF2 on high graphic settings, 4xAA, no v-sync, ….
I’ve always had ~200 FPS (playing with FPS_max 150) but since i few days i randomly get “Lag-spikes”. Even though i have at least 50-60 FPS at any time (randomly drops down to 50 and goes back up again during mid-fights for example [Note: That already happened before]), good/consistent ping and 0 choke, it feels like playing with 200 ms Ping half of the time. Epic rocket-lag and teleporting players included.
It doesnt matter if i play on low or high settings – for some unknown reasons i always have the same FPS…
I really dont know how to fix this… I tried changing the net settings and the priority of TF2 in the task-manager, but that didnt help either. I only have that problem in TF2, MW2 runs smooth on max settings all the time for example.
Few specs:
ATI Radeon 4890 HD 1GB
AMD Phenom X4 II 955
ASRock M3A790GXH
4GB DDR3 Ram
Any ideas? :/
TIA

Can you show us your net_graph 3? or tell us if you have loss or choke?

This is how my net_graph looks like during these weird lags. Choke was usually between 1 and 8 on this pub server.
I also have this problem on bball servers with just 4 players. Not as noticable as during pcws, but still a lot of teleporting players. Note: I get 1 or 2 choke max on the bball servers.

Are you running wired or wireless?
Wired.

Edit: Might take a look at this ping test, bad result: http://www.pingtest.net/result/19123670.png

Speedtest results look fine. Try doing a pingtest to a server in the same country as the server you mainly experience these problems on.

Already edited the ping test into my other post.
This is a ping test to the nearest city available: http://www.pingtest.net/result/19123670.png
Wow, that packet loss is really bad.
Try doing it to a different nearby server (not the one you just tested with) and paste that result.

Ping test to a server in Prague (same result): http://www.pingtest.net/result/19123958.png

Right, let’s try and pinpoint where that issue occurs.
Please download winmtr from http://winmtr.sourceforge.net/, start pinging the IP of a server which you have this problem on, and run it until Sent reaches 100, then click ‘copy as text’ and paste it here.
could it just be that he has a really bad cable? i don’t know much about this stuff. But trying a different cable can do no harm
Everytime i enter an IP and click on “Start” it says “Unable to get raw socket” and stops it, how can i fix that?
As you have probably noticed by now, english isn’t my primary language, so sorry if this is some kind of “L0L NUB GTFO” question. :P
And i bought a new cable 2 months ago, so i dont think that it’s my cable…

Unlikely.

Oh, it doesn’t work on Windows 7, sorry.
Try with tracert, but it won’t give out the same quality of information.
Open a command prompt and enter the following (after the new line you should press enter to run the command)
cd \
tracert IP > etf2ltracert.txt (replace IP with the IP of the server)
then paste what is in c:\etf2ltracert.txt here. Don’t worry if it seems to have hung, just leave it for a minute or so and you should see a new line appear eventually. Give it 2 minutes to run.
edit: also i’ve not noticed any mistakes with your english, so don’t worry about it :D
