VAC2 is always loaded on game start, though the detections are random, you could have a detected cheat and not get detected.
VAC3 is loaded randomly while you play, you could play for hours without it getting loaded.

It’s pretty random, if you’re lucky you could have a detected cheat and not get detected at all.
